Alzheimer’s Disease Program Initiative (ADPI) "Seeking Hope"

 

WHAT IS DEMENTIA?

It is the loss of cognitive functioning—thinking, remembering, and reasoning—to such an extent that it interferes with a person’s daily life and activities.  Please click on this link to view video: WHO: Dementia - it affects us all.

DID YOU KNOW?

Census estimates that 3 in 9 Native American Elders aged 65+ will develop dementia.  And, the number of Native Americans aged 65+ living with memory problems is projected to grow over 5 times in the next 4 decades.

“SEEKING HOPE”

Seeking Hope provides quality, person-centered care and support for Native Americans.  Certified Dementia Specialists are available to deliver Home and Community Based Services to people living with dementia and their family members.  These services consist of caregiver support trainings; dementia-specific consultation, resource assistance and material; and assistance with home care planning.

Two programs are available for caregivers and family members of those who are experiencing memory loss.

MUSIC & MEMORY

Music enhances the lives of people living with dementia and has been shown to improve mood and increase physical activity.  The Seeking Hope program provides portable music players to individuals with memory loss.  The portable music players will house the individual’s favorite songs, a process that is done either by a Dementia Care Specialist or the individual’s family or friends.

The Seeking Hope Program will provide eligible participants with:

  • A music player, headphones, Bluetooth speaker, and more.
  • An assessment to determine music preferences.
  • A home visit by a Dementia Care Specialist to instruct the participant and/or the caregiver on how to use the equipment and download music.
  • Follow-up to evaluate the program and to assist with playlist or equipment.

SAVVY CAREGIVER IN INDIAN COUNTRY

Savvy Caregiver is a program for Native American people and is designed to help caregivers provide care and support for someone with Alzheimer’s or dementia.  It is a six-session program which lasts approximately two-hours each session.  This free program offers strategies to providing care, resources and support to the caregiver and are critical to maintaining a high quality of life for both the person living with dementia and the people caring for them.

This training will help you:

  • Have a better understanding of dementia, the different stages and behaviors.
  • Recognize & develop self-care strategies.
  • Identify resources that can help you with managing care.
  • Learn how to address challenging behaviors & keep a manageable routine.
  • Learn from the comfort of your own home, on-line or in a group setting.

Our Dementia Care Specialist will work with you and family to assist in getting the right information, resources, connections, and address your growing concerns.  It will help you and your loved ones to plan for the future in many ways.
